LB-65 Compatibility Testing

بسم الله الرحمن الرحيم 
 The transfusion services establish a process for compatibility testing.
LB.65.1 There is a process to ensure the detection of ABO incompatibility between the recipient’s serum/ plasma and the donor’s RBC.
LB.65.2 The process ensures the compatibility testing is performed on integrally attached segment from the donor’s RBC unit.
LB.65.3 The process ensures the checking for presence, now or in the past, of clinically significant antibody in the patient’s serum.
LB.65.4 The process ensures the proper labeling of cross-matched units with patient’s name, patient’s identification number and patient’s ABO /Rh-D.
معيار اليوم هو اخر نقطه لتتويج عملنا السابق ببنك الدم حيث هيتم تحضير وحده دم مناسبه لمريضك 
وهنا يستلزم ان تكون في سياسه واضحه لطريقه عمل compatibility  testing وبالتالي هيكون في lab test procedure 
يوضح هذا الامر بالخطوات ومنها
١-عمليه التوافق بين 
patient  plasma or serum
 علي حسب طريقه الاختبار وما تحدده الشركه     
                        و
 Intact Segment RBCs 
بمعني تكون مفصوله من الوحده لهذا الغرض فقط ووقت عمل التوافق في نفس اللحظه
٢-التأكد من عمل antibody screening  كما شرحناه في LB-64
٣-طريقه ومده حفظك للوحده التي تم عمل التوافق معها 
وهذه الطريق بتكون اسهل في وجود computer system 
ولكن في عدم وجوده يستلزم ربط request لطلب نقل الدم بالوحده التي تم التوافق معها او وضع sticker او .....
هيدخل الكثير من policies  كَ cross reference  policies 
منها 
-unit labeling 
-Patient identification

-Antibody screening 
-Blood grouping. 
-transfusion guideline 
لأنك هتحدد للطبيب انك هتحتفظ بcross matched unit لمده 72ساعه فقط وبعدها هيكون من حقك ان تفك الوحده لمريض اخر

بسم الله الرحمن الرحيم  ا The laboratory has a system for personnel audit trail. LB.4.1 The system allows for the identification of who performed a critical task/step. LB.4.2 The system allows for the identification of when, where, and why the task/step is performed . اليوم  مع  سيادتكم هنضع مبدأ نمشي عليه السبعين معيار القادمين باذن الله ويكون  بمثابة اتفاق الا هو .........مين ؟؟؟؟؟ عمل ايه؟؟؟؟؟؟ 😃😃😃 هي صغيره  في نظر بَعضُنَا ولكنها كبيره جدا جدا وقت الحساب (وقت المشاكل لا قدر الله ) المعني :  ان النظام القائم بالمختبر يستطيع أن يتبين مين اللي عمل الخطوه المعينه  في اي وقت وفِي اي مكان......................وهذه  سهله الي حد ما  في وجود نظام حاسب آلي  LIS  يحكم عمليات المختبر ولكن عدم وجود مثل هذا النظام ,يتطلب مجهود كبير......... اولا :فى حالة وجود الكمبيوتر : .فيكون  الآتي: نشيك علي كل أعضاء المختبر ان يكون لهم p assword خاص بيهم والعمل علي ان كل موظف  لايستعمل Password الآخرين .........واعمل تفتيش دوري علي المختبر .........للتأكد من أ،  كل شخص لايفرط فيه و

بسم الله الرحمن الرحيم The laboratory has a system for the receipt of incoming supplies and   services, inventory management, and tracking of critical materials . LB.6.1 The laboratory implements policies and procedures on documenting the receipt, inspection, and testing (as applicable) of incoming critical material or service. LB.6.2 The laboratory implements policies and procedures on inventory management and tracking the use of critical materials, supplies, and reagents to ensure the followin g :   LB.6.2.1 Materials are used within their expiration dates. LB.6.2.2 New reagents lot numbers are tested against old lots or suitable reference materials before use . LB.6.2.3 Kit components are used within the kit lot number .
