LB-17 Safety & Infection Control

17 The laboratory has a comprehensive safety and infection control programs.
LB.17.1 The laboratory implements safety and infection control policies and procedures that are in
compliance with the national and international laboratory safety standards as well as the hospital safety and infection control plan. Policies define the following 
LB.17.1.1 Chemical hygiene plan.
LB.17.1.2 Mercury reduction/elimination plan.
LB.17.1.3 Mechanism of fumes and vapors monitoring.
LB.17.1.4 Mechanism of compressed and flammable gases control.
LB.17.1.5 Radiation safety plan.
LB.17.1.6 Biological safety procedures and use of standard precautions.
LB.17.1.7 Tuberculosis and other biological hazards exposure plan.
LB.17.1.8 Electrical safety plan.
LB.17.1.9 Fire prevention and control plan.
LB.17.1.10 Provision and use of Personal Protective Equipment (PPE)

LB.17.1.11 Provision and control of negative pressure in sections dealing with highly infectious

materials.

LB.17.1.12 Provision, use, and control of fume hoods.
LB.17.1.13 Provision, use, and control of biological safety cabinets.
LB.17.1.14 Provision of safety equipment (eye wash, emergency shower, fire extinguisher, fire blanket, biological and chemical spill kits  

LB.17.1.15 Waste disposal/control plan (chemical, biological and sharps) using prick proof

containers and leak proof bags.

LB.17.1.16 Provision and use of first aid kits.
LB.17.1.17 Reporting of infection and safety incidents.
LB.17.2 The laboratory has safety and infection control training program that includes:
LB.17.2.1 Initial training and competency assessment.
LB.17.2.2 Annual training, recertification and competency assessment.
LB.17.3 The laboratory has a system for monitoring the laboratory safety and infection control program.
LB.17.3.1 Documented safety and infection control audits are conducted at regular predefinedintervals
(at least twice yearly).
LB.17.3.2 Findings of the audits are reported to the laboratory director, the facility safety officer,
the infection control department, and other concerned parties.

موضوع safety' and infection control  موضوع كبير  
مطلوب
أولا :  سياسه عمل توضح النقاط الاتيه ومعها خطه تدريبية للعاملين الجدد وهذه كانت احدي النقاط في orientation program  ثم مايثبت انهم قادرين علي تنفيذ سياسه العمل وذللك ب competency  checklist  سنويا وما يتطلب من corrective action 
كما يطلب منك المراقبه المستمره والدوريه بما يسمي safety auditing  report وهو دوره تفقدية  لمسءول الأمن والسلامه أسبوعيا ........وما يتبعها من تقرير كما يتطلب وجود إشارات الأمن والسلامه وأدوات تساعد عليه 
ثانبا :  محتاجين شخص موءهل لذلك يكون قد أخذ اي دوره تدريبية علي safety  &IQ  وعلي علم بما يسميOSHA&NFPAلان هذه ألمنظمات  هي reference  حيث ان سباهي سرفير بيطلبوا الملف الشخصي لهذا المسءول ويطلعوا علي job description وموجود كأحد أضلاع organization chart المتعلق فى كل مكان  وكل موظف فى المختبر يعرف مين هو ..........وكيفية الاتصال بيه
النقاط هى 
اولا  chemical hygiene 
بما انا في مختبر ...فاكثر  ناس بتتعامل مع المواد الكمياءيه  وخصوصا الهستوباثولوجي فأنا باقترح تخصيص احد أفراد المختبر ليكون( chemical hygiene personal (CHP)بفضل الله طبقت هذا عندي وكان تطبيق موفق.... وأعجب بها  السرفيير جدا وكانت مهامه الآتي 
MSDS 
تكلمنا  قبل كده ان كل محلول ليه Material safety data sheet ولازم الشركه تمدنا بيه او ندخل علي النت هنلاقي مواقع بتمدك بيه وهو بيتكلم علي مدي خطوره الماده من حيث الإشعاع او الاحتراق او الانفجار .... وغيرها وبيكون مرسوم عليها مدي خطورتها 1-طرييقه التخزين الأمن 
2-طريقه التعامل معها اذا انسكبت علي الارض او لامست الجلد او العين 
ومن هنا عملت risk assessment form

فالامر طويل وشاق ولكن خطوه خطوه  ابتديت ب 
١-كل وحده مسءوله انهاتعطي Material safety data sheet وتسلمه للchp
٢-ويجمعها alphabetical لهذه الوحده (فقط)

٣-ويقوم بعمل جدول يقسم فيه المواد  كما سبق

٤-ثم يشرح للموظفين  بالوحده النقاط السابقة 
(طبعا لن يستطيع اي احد ان يحفظ كل MSDS)
لكن وجدنا عامل مشترك الا وهو ان معظم المحاليل المستخدمة تشبه بعض في خطورتها وطريقه التعامل معاها وتخزينها 
فكانت مهمه chp انه يجمع الملخص المفيد بمعني يعلم الشباب ان مثلا كل شي ء بنتعامل معاه...........الا المحلول
 ....بنتعامل معاه ....... 
والمحتاج لتفاصيل  اكتر بيرجع للجدول  ويعرف كل واحد في القسم فين MSDS 
يعلمه الجدول بما فيه من رموز ثم يعمل له risk assessment form ولو تتذكروا دا كان احدي نقاط orientation program ومكونات personal file ....
*الارشادات موجودة فى كل مكان لمعرفة العاملين بيها ...

وبعد risk assessment form يتعمل competency ضمن Safety competency form annually

ولضمان التاكد من أن كل شخص بالمختبر تم عمل له Competency   الخاص به سنويا  أقترح عمل الكارد التالى : حيث يهدف الى معرفة موعد كل شخص بالتقييم والنقاط الاساسية به :

LB-4 Personal Audit Trail

بسم الله الرحمن الرحيم  ا The laboratory has a system for personnel audit trail. LB.4.1 The system allows for the identification of who performed a critical task/step. LB.4.2 The system allows for the identification of when, where, and why the task/step is performed . اليوم  مع  سيادتكم هنضع مبدأ نمشي عليه السبعين معيار القادمين باذن الله ويكون  بمثابة اتفاق الا هو .........مين ؟؟؟؟؟ عمل ايه؟؟؟؟؟؟ 😃😃😃 هي صغيره  في نظر بَعضُنَا ولكنها كبيره جدا جدا وقت الحساب (وقت المشاكل لا قدر الله ) المعني :  ان النظام القائم بالمختبر يستطيع أن يتبين مين اللي عمل الخطوه المعينه  في اي وقت وفِي اي مكان......................وهذه  سهله الي حد ما  في وجود نظام حاسب آلي  LIS  يحكم عمليات المختبر ولكن عدم وجود مثل هذا النظام ,يتطلب مجهود كبير......... اولا :فى حالة وجود الكمبيوتر : .فيكون  الآتي: نشيك علي كل أعضاء المختبر ان يكون لهم p assword خاص بيهم والعمل علي ان كل موظف  لايستعمل Password الآخرين .........واعمل ...

LB.6.1 The laboratory implements policies and procedures on documenting the receipt, inspection, and testing (as applicable) of incoming critical material or service. LB.6.2 The laboratory implements policies and procedures on inventory management and tracking the use of critical materials, supplies, and reagents to ensure the followin g :   LB.6.2.1 Materials are used within their expiration dates. LB.6.2.2 New reagents lot numbers are tested against old lots or suitable reference materials before use . LB.6.2.3 Kit components are used within the kit lot number .
