LB-75 Histo-pathology Report

بسم الله الرحمن الرحيم 
The anatomical pathology develops guidelines for compiling surgical pathology reports.
LB.75.1 There are implemented guidelines for compiling surgical pathology reports, addressing the following elements:
LB.75.1.1 Gross description (type, number, dimensions).
LB.75.1.2 Essential processing information and performed studies.
LB.75.1.3 Other relevant report elements necessary for the management of the patient.



كمثل اي قسم من الأقسام بينتهي بتقديم النتيجه او التقرير علي الاختبار او الخدمه المقدمه ...ومثل ماحدث تماما في 
LB-2 4
بنفس المواصفات بنكتب سياسه عمل بتوضح فيه  مكونات تقرير الهستو 

وبتضع فيه سياسه test result reporting ك cross  reference  policy  ثم يزيد ويؤكد علي report الخاص ب histopathology 
اولا :الوصف التفصيلي للعينه من حيث 
نوعها 

عددها 

مقياسها 

ثانيا:المعالجات التي تمت للعينه والاختبارات التي أضيفت اليها  مثل :
Microscopic examination


Especial stains

Immuno-histochemistry .....,,,,
وبالتالي كل هذا موجود عندك في سياسات  العمل  ك LTP...lab test procedure وتضعها  هنا .....ك cross reference policies 
-وإذا كان في fully automated machine للصبغه مثلا بينطبق عليها كل ماسبق  من validation الي Qc....
وبالتالي بتكون مجهز كل شيء عنها 

ثالثا : يحتوي التقرير علي بعض التوجيهات التي تتماشي مع الحاله 
مثال 
another sample for  margins -
-Follow up by mammogram 
CT guided biopsy 
Cytogenetic studies 
Clinical Correlation ...........,وهكذا

بسم الله الرحمن الرحيم     LB.1.1 The laboratory has a clearly defined scope of services. LB.1.2 The laboratory services are in compliance with applicable national standards. بسم الله الرحمن الرحيم  ا The laboratory has a system for personnel audit trail. LB.4.1 The system allows for the identification of who performed a critical task/step. LB.4.2 The system allows for the identification of when, where, and why the task/step is performed . اليوم  مع  سيادتكم هنضع مبدأ نمشي عليه السبعين معيار القادمين باذن الله ويكون  بمثابة اتفاق الا هو .........مين ؟؟؟؟؟ عمل ايه؟؟؟؟؟؟ 😃😃😃 هي صغيره  في نظر بَعضُنَا ولكنها كبيره جدا جدا وقت الحساب (وقت المشاكل لا قدر الله ) المعني :  ان النظام القائم بالمختبر يستطيع أن يتبين مين اللي عمل الخطوه المعينه  في اي وقت وفِي اي مكان......................وهذه  سهله الي حد ما  في وجود نظام حاسب آلي  LIS  يحكم عمليات المختبر ولكن عدم وجود مثل هذا النظام ,يتطلب مجهود كبير......... اولا :فى حالة وجود الكمبيوتر : .فيكون  الآتي: نشيك علي كل أعضاء المختبر ان يكون لهم p assword خاص بيهم والعمل علي ان كل موظف  لايستعمل Password الآخرين .........واعمل تفتيش دوري علي المختبر .........للتأكد من أ،  كل شخص لايفرط فيه و

بسم الله الرحمن الرحيم The laboratory has a system for the receipt of incoming supplies and   services, inventory management, and tracking of critical materials . LB.6.1 The laboratory implements policies and procedures on documenting the receipt, inspection, and testing (as applicable) of incoming critical material or service. LB.6.2 The laboratory implements policies and procedures on inventory management and tracking the use of critical materials, supplies, and reagents to ensure the followin g :   LB.6.2.1 Materials are used within their expiration dates. LB.6.2.2 New reagents lot numbers are tested against old lots or suitable reference materials before use . LB.6.2.3 Kit components are used within the kit lot number .
