LB-30 Sample Retention System

.30 The laboratory has a system for sample retention.
LB.30.1 There is a sample retention policy to ensure that general laboratory specimens are retained under appropriate conditions for no less than the periods specified below:
LB.30.1.1 Whole blood specimens and urine specimens are retained for twenty four hours.
LB.30.1.2 Serum, plasma, cerebrospinal fluid and other body fluids specimens are retained for forty eight hours.
LB.30.1.3 Permanently fixed and stained blood films are retained for seven days.
LB.30.1.4 Permanently fixed and stained microbiology slides are retained for seven days.
LB.30.2 The sample retention policy ensures that donors and patients samples are retained under appropriate conditions for no less than the periods specified below:
LB.30.2.1 Outpatient specimens (not for compatibility testing) are retained for twenty four hours.
LB.30.2.2 Inpatient specimens are retained for seventy two hours.
LB.30.2.3 Specimens of patients who receive blood transfusion are retained for seven days after transfusion.
LB.30.2.4 Segment/specimens from transfused RBC are retained for seven days aftertransfusion.
LB.30.2.5 Specimens for transfusion reaction investigation are retained for seven days.
LB.30.3 The sample retention policy ensures that anatomical pathology specimen are retained under appropriate conditions for no less than the periods specified below 
LB.30.3.1 Gross specimens of wet or fixed tissues are retained for fourteen days after therelease of final report.
LB.30.3.2 Paraffin blocks are retained for ten years.
LB.30.3.3 Glass slides are retained for ten years

مثل مااهتمت  معايير سباهي لوضع نظام وقتي لحفظ الملفات وهومايضمن حقوق الموظفين وضعت نظام وقتي لحفظ العينات وهو لحفظ حق الموظف كما يتيح للطبيب ان يطلب تحليل اخر من نفس العينه( ان أمكن وعلي أسس علميه)
وبتالي فان السياسه الأولي سياسه داخليه IPP
والاخري يجب ان يعرف بها الطرف الاخر (فيمكن ذكرها بجدول في (laboratory service guide )يعنى APP
قبل مانتطرق لهذا الجدول نذكر الآتي 
A-ان مكان ودرجه الحراره تخزين كل عينه يرجع الي الاختبارات المطلوبه للمريض كما بين    pamphlet  من حيث   : 
 room temperature-     أو -2...8Cفي ثلاجه .........أو --18في فريزر
B-وبالتالي يجب ترتيب مكان التخزين من حيث 
١-يكون هناك مكان مخصص للعينات  وموجود مايشير ان هنا سpatients sample 
حتي ولو كان عندك ثلاجه واحده، خصص رف ويكتب عليه 
٢-ترتيب العينات بطريقه يسهل بها الوصول للعينه المطلوبه او التخلص من العينات اللي جاء وقت التخلص منها .....مثلا ترتيب بأيام الاسبوع 
٣-كما تنبه علي الفنيين ان يرتب العينات داخل racks

٤- ووجود temperature check list كل ٨ساعات علي الثلاجه او الفريزر 
٥- المحافظة علي نظافه المكان بمسح الثلاجه من الداخل مره كل شهر بيكربونات الصوديوم كما ذكرنا 
وبالتالي يمكن ان تضع 
monitoring of temperature and  maintenance  of freezer fridge  As Cross reference  policies
C-اما من حيث النظام الوقتي  للاحتفاظ بالعينات فنصت علي : 
١-المختبر ككل العينات ...٤٨ ساعه ............................... الا البول ......٢٤ ساعه

٢-أفلام الميكرو.  والهيماتولوجي ......٧ ايام 

٣-بنك الدم 
ا- عينه cross match لعمل cross match اخر ....٧٢ساعه وهنا بيحبوا السرفير يسألوا الفنيين ليه ليه......؟؟؟؟😁😁😁
ب-نفس العينه اللي فاتت يحتفظ بها أسبوع بعد نقل الدم مع segment لوحده او وحدات الدم المنقولة 
ج-لوحدث لأقدر الله adverse reaction يحتفظ بالعينات والوحده او segment منها ........لأسبوع اخر 


٤- الباثولوجي 
-البلوك والافلام .........10سنين 
-الأنسجة نفسها (العينه) .....١٤ يوم تحتسب من خروج النتيجه

بسم الله الرحمن الرحيم  ا The laboratory has a system for personnel audit trail. LB.4.1 The system allows for the identification of who performed a critical task/step. LB.4.2 The system allows for the identification of when, where, and why the task/step is performed . اليوم  مع  سيادتكم هنضع مبدأ نمشي عليه السبعين معيار القادمين باذن الله ويكون  بمثابة اتفاق الا هو .........مين ؟؟؟؟؟ عمل ايه؟؟؟؟؟؟ 😃😃😃 هي صغيره  في نظر بَعضُنَا ولكنها كبيره جدا جدا وقت الحساب (وقت المشاكل لا قدر الله ) المعني :  ان النظام القائم بالمختبر يستطيع أن يتبين مين اللي عمل الخطوه المعينه  في اي وقت وفِي اي مكان......................وهذه  سهله الي حد ما  في وجود نظام حاسب آلي  LIS  يحكم عمليات المختبر ولكن عدم وجود مثل هذا النظام ,يتطلب مجهود كبير......... اولا :فى حالة وجود الكمبيوتر : .فيكون  الآتي: نشيك علي كل أعضاء المختبر ان يكون لهم p assword خاص بيهم والعمل علي ان كل موظف  لايستعمل Password الآخرين .........واعمل ...

LB.6.1 The laboratory implements policies and procedures on documenting the receipt, inspection, and testing (as applicable) of incoming critical material or service. LB.6.2 The laboratory implements policies and procedures on inventory management and tracking the use of critical materials, supplies, and reagents to ensure the followin g :   LB.6.2.1 Materials are used within their expiration dates. LB.6.2.2 New reagents lot numbers are tested against old lots or suitable reference materials before use . LB.6.2.3 Kit components are used within the kit lot number .
